Here at First Step we are delighted to launch our Case Management and Wellbeing program. Thanks to the generosity of the Asian Australian Foundation, the City of Port Phillip and the Marian EH Flack Trust, the clinical team at First Step will now include 2 days per week of case management assisting clients with the coordination of care, counselling, support around other issues such as homelessness and employment, and other factors in their lives. Non-judgemental, wrap-around care will be facilitated for eligible clients by case managers Dom Behan (pictured) and Lauris Hanlon.
Case Management and Wellbeing will be targeted at First Step clients who are homeless or at risk of homelessness. Clients will be proactively assisted over 4 half-days, and at least one of the Case Management team staff will be at the clinic at all times to manage drop-ins and crisis.
First Step would like to thank the Asian Australian Foundation, the City of Port Phillip and the Marian EH Flack Trust for supporting the first 12 months of this crucial work. Homelessness is arguably the single most disempowering factor in a vulnerable person's life, and this program will assist First Step to overcome that barrier and ensure the everybody has every chance to turn their lives around.
